There are three ways to join the Coalition: PRACTICE, ACTIVE and ASSOCIATE.
Only one active, voting member per practice. Dues are $250/year for a practice membership (includes two (2) free at the conference and entire practice participation in webinars and list serv). Additional members, more than two (2), attending conference are $50 each.
Only one active, voting member per practice. Dues are $75/year for first, voting member of a practice, $50/year additional member.
Individuals working within an Oncology Hematology Clinic but the Clinic is already represented by an Active Member.
Associate members benefit from an association designed to help individual oncology professionals learn from their peers and share ideas, advice, and experience. Associate members are eligible to attend CHOP business and educational meetings (1 regional and 1 state annually).

Please Note: All CHOP membership runs on the fiscal year of January 1st – December 31st. All membership fees are subject to change and due when the application is submitted, and non-refundable, if application is approved. Individual memberships are non-transferable.
