Dear Healthcare Provider,
As you might know, the Genentech® Access to Care Foundation (GATCF) recently changed its eligibility criteria. The changes to the criteria were recently communicated to your practice via a letter from GATCF, but I wanted to ensure you received this information.
In order to reach more patients who do not have access to Genentech medicines, GATCF now considers certain patients whose out-of-pocket costs for their Genentech medicines account for greater than or equal to 10% of their annual household adjusted gross income (AGI) for program eligibility. Certain financial and medical criteria must also be met. Specific criteria changes can be found in the attached document.
